В монографии на основании анализа литературных данных и критической
оценки существующих методов лечения детей с заболеваниями и повреждениями
селезенки, большого числа собственных клинических наблюдений и экспериментальных
данных обоснована хирургическая тактика при травмах, кистах,
опухолях и других заболеваниях селезенки. Предложены оригинальные оперативные
приемы, позволяющие заменить спленэктомию органосохраняющими
операциями, в том числе резекцией селезенки с сохранением ее верхнего полюса
на коротких желудочных сосудах. Авторами составлена программа ведения детей
с травмой селезенки, что позволяет излечить большую часть пострадавших консервативно.
Монография
рассчитана на детских хирургов, хирургов, педиатров, травматологов,
врачей скорой медицинской помощи.

The summary. In this book authors suggested the tactics at cases of traumas, cysts, tumors
and other diseases of a spleen; as well as the original surgical techniques, allowing to replace
splenectomy by the organ-preserving operations, including the original method of
spleen resection developed by the authors. The content is based on the analysis of the
literary data and a critical estimation of existing methods of treatment of children with
diseases and damages of a spleen, the big clinical experience and experimental data with
use of modern research methods. Authors suggested the tactics in case of splenic trauma
which allows to cure the most part of traumatized children without surgery.
The book is designed for pediatric surgeons, general surgeons, pediatricians, trauma
surgeons, emergency care specialists.

PREFACE
Pediatric surgery is a very rapidly
developing specialty. But sometimes
it is easier to do as it was used
for many years before and we are
not free of the stereotypes. Among
the numerous problems of pediatric
surgery the surgery of spleen takes
a special place, because in case of
blunt abdominal trauma in children
the spleen is injured more often than
the other organs. In case of splenic
trauma the majority of Russian surgeons
consider the splenectomy to be
the method of choice. This stereotype
is very common. But removal of the
spleen leads to the number of significant
and sometimes fatal complications.
It is a stimulus for search of the
new ways of splenic preservation and
it had forced us to describe our tactics
in this book. The purpose of Surgical
Diseases and Trauma of Spleen
in Children is to present to Russian
surgeons the modern trends in operative
and non-operative treatment,
the review of last publications and the
results of the authors research. Surgical
Diseases and Trauma of Spleen in
Children is divided into six parts. The
first part, Anatomy and physiology of
the spleen, contains the description
of splenic anatomy in children, its
histology, blood supply and innervation.
The questions of the following
splenic functions are also covered: hemolytic,
hemostatic, metabolic, and
hormonal. The role of the spleen in
immunity is described in details. The
second part, History of treatment of
splenic lesions and injuries, contains
the short history of conservative and
operative treatment of splenic trauma
and lesions since 1549; changes in
the attitude to splenic functions from
Middle Ages up to now, and the modern
trends.
In the third part, Blunt splenic
trauma, general tactics in trauma
is addressed. This chapter contains
the data about the patients admitted
to the Vladivostok University
Children’s Hospital since 1979 to

2002 with blunt splenic trauma. It
contains the analysis of frequency,
sex and age, combination of splenic
trauma to the other organs. Dependencies
on a season and the daytime
are determined. Also the typical
signs and symptoms, anamnesis
are described. The laboratory data
is analyzed. Indications and contraindications
to various diagnostic
methods, their specificity and
reliability are determined, possible
complications are described. The
following methods were estimated:
ultrasonography, radiographic
methods including angiography, CT,
scintigraphy, DPL, laparoscopy etc.
The general characteristics of the
methods of treatment are resulted
and own data and own tactics are
described. The forth chapter, Surgical
techniques, contains the description
of typical surgical procedures on
spleen and indications, contraindications,
dangers, mistakes, complications.
The following methods are
described: splenorrhaphy, splenectomy
and partial splenectomy, autotransplantation
of splenic tissue etc.
Indications and contraindications to
splenic artery embolization are determined.
The own data is resulted; the

complications are described and discussed.
In addition, this chapter contains
experimental and clinical data
of splenic resection with preservation
of the upper pole in case of splenic artery
supply absence. Clinical features,
diagnostics and general tactics in the
case of the different splenic lesions
are covered in the fifth chapter. Frequency,
typical features, diagnostics
and treatment of various splenic congenital
anomalies, splenic cysts, dystopia,
and tumors are discussed. This
chapter also contains indications to
different procedures on spleen in case
of hematological disorders and portal
hypertension and their possible complications.
The last chapter is devoted
to short- and long-term complications.
All complications are divided
to intraoperative, postoperative and
complications after non-surgical
treatment; local and general complications,
specific and non-specific
etc. The chapter also contains the
description of the ways of prevention
of specific complications. In conclusion
this research and overview had
shown the possibility of significant
restriction of surgical activity in children
with blunt splenic trauma for the
benefit for conservative therapy.

ВВЕДЕНИЕ
Среди многочисленных проблем
хирургии детского возраста
хирургия селезенки занимает
особое место. Основным методом
лечения кист, опухолей
селезенки, различных гематологических
заболеваний (идиопатическая
тромбоцитопатия,
гемолитическая анемия и др.)
является спленэктомия. При повреждениях
селезенки, занимающих
у детей 1-е место среди повреждений
внутренних органов
при закрытых травмах брюшной
полости, общепринятой операцией
большинство хирургов
считает спленэктомию. До сих
пор многие практические врачи
показания к спленэктомии
основывают, согласно Р. Шерману
(1984), на четырех ложных
предпосылках:
1) селезенка не является органом,
необходимым для существования
организма;
2) селезеночная ткань не может
заживать спонтанно;
3) двухмоментные разрывы селезенки
фатально опасны;
4) количество осложнений
после консервативного лечения
превышает количество таковых
после спленэктомии.
Между тем селезенка – далеко
не лишний орган. Она играет
огромную роль в организме человека
и, что важно, особенно в
организме растущего ребенка,
являясь иммунокомпетентным
органом. Иммунная роль селезенки
не подлежит сомнению, в
настоящее время убедительно доказано,
что ее отсутствие может
привести к ряду тяжелых и иногда
фатальных осложнений.
Удаление селезенки далеко не
безразлично для жизнедеятельности,
и принятие этого факта
за постулат служит стимулом для
поиска новых путей полного или

частичного сохранения этого органа
в случаях необходимости
спленэктомии, то есть замены
радикальной операции органосохраняющими
методами лечения,
что и заставило нас заняться
изучением данного вопроса.
